Parkinson's Disease and Therapy — Why Early and Ongoing Treatment Changes Everything
A Parkinson's disease diagnosis brings with it a wave of questions, fear, and uncertainty. One of the most common things we hear from newly diagnosed patients and their families is this: "We weren't told therapy was an option. We thought we just had to wait and see."
That waiting is one of the most significant missed opportunities in Parkinson's care.
The research is clear: early, intensive, specialized therapy for Parkinson's disease slows the progression of symptoms, preserves function longer, and meaningfully improves quality of life. And at Salience NeuroRehab, we offer more Parkinson's-specific programs than any other clinic in Southern Indiana.
How Parkinson's affects the body
Parkinson's disease is a progressive neurological condition caused by the loss of dopamine-producing neurons in the brain. Dopamine is a neurotransmitter that plays a central role in coordinating smooth, controlled movement. As dopamine levels decline, the characteristic symptoms of Parkinson's emerge — tremor, rigidity, bradykinesia (slowness of movement), postural instability, and changes in speech and swallowing.
Over time, movements become smaller and slower. Steps get shorter. Handwriting shrinks. The voice becomes softer and harder to understand. Swallowing becomes effortful. And the instinct to compensate for these changes — by moving less, speaking less, doing less — can actually accelerate the progression of the very symptoms the individual is trying to manage.
This is where therapy intervenes.
LSVT BIG — training the brain to move bigger
LSVT BIG is an evidence-based, intensive physical and occupational therapy program developed specifically for individuals with Parkinson's disease. The program is built on a single powerful principle: that individuals with Parkinson's have lost the internal sense of how large their movements actually need to be. Their brain tells them they are moving normally when in reality their movements have become small, slow, and effortful.
LSVT BIG retrains the brain to produce big, full, exaggerated movements — and through consistent high-repetition practice, those bigger movements become the new normal. The research shows improvements in walking speed, stride length, balance, arm swing, posture, and ability to perform daily living tasks.
The program consists of four sessions per week for four weeks — 16 sessions total — and the results are most significant when started early in the diagnosis. At Salience, our LSVT BIG certified therapists have delivered this program with outstanding results and are among the few certified providers in the region.
Rock Steady Boxing — fight back against Parkinson's
Rock Steady Boxing is a non-contact, boxing-inspired fitness program designed specifically for people with Parkinson's disease. It is not a gentle exercise class — it is an intense, energizing, empowering workout that research has shown to improve strength, balance, agility, hand-eye coordination, and cardiovascular fitness in individuals with Parkinson's.
The physical benefits are significant. But the community aspect of Rock Steady is equally powerful. Participants train alongside others who understand what they are going through. They encourage each other, push each other, and show up for each other. For many participants it becomes the most important hour of their week.
At Salience, our certified Rock Steady coach runs classes in partnership with the Tri-County YMCA on Tuesdays and Thursdays from 10:00 to 11:00 AM. For individuals who cannot attend group sessions, Rock Steady is also available as an in-home program.
SPEAK OUT!® — protecting your voice before it fades
One of the most distressing and often under-addressed symptoms of Parkinson's disease is the gradual loss of vocal volume, clarity, and swallowing safety. Many individuals and their families notice the voice getting softer, harder to understand on the phone, or trailing off at the end of sentences — often years before it becomes a serious concern.
SPEAK OUT!® is an evidence-based speech therapy program developed by the Parkinson's Voice Project that directly addresses this progression. The program teaches individuals to speak and swallow with conscious intent — converting what has become an automatic, diminished function into a deliberate, engaged one. This neuroplasticity-based approach produces measurable improvements in vocal volume, speech clarity, and swallowing safety.
Our speech and language pathologist is the only SPEAK OUT!® certified provider in the area and has delivered this program with consistent success. We strongly encourage individuals with Parkinson's to begin voice therapy early — before the changes become significant — because it is far easier to maintain function than to rebuild it.
You don't have to wait for symptoms to get worse
The most important thing we want every person with a Parkinson's diagnosis — and every family member supporting them — to know is this: therapy is not something you wait for until things get bad. It is something you start now, while there is the most to gain and the most to protect.
Salience NeuroRehab offers LSVT BIG, Rock Steady Boxing, and SPEAK OUT!® — the three most evidence-based specialized Parkinson's programs available — under one roof, in Jasper, Indiana. We also offer mobile therapy for individuals who find clinic attendance difficult.
If you or someone you love has been diagnosed with Parkinson's disease, call us at 812-998-6176 or request an appointment online. The sooner therapy begins, the more it can do.
