When people hear that Salience NeuroRehab offers therapy in the home, the first assumption is often that we are a home health service. We are not — and the distinction matters more than most people realize.

Understanding the difference between home health and mobile outpatient therapy could open up options for your care that you didn't know existed.

What is home health therapy?

Home health physical, occupational, and speech therapy is provided to individuals who meet a specific Medicare and insurance requirement: they must be considered "homebound." Homebound status means that leaving the home requires a considerable and taxing effort — essentially, that you are unable to safely or reliably leave your home for routine activities.

Home health is an important and valuable service for individuals who genuinely cannot leave home. But the homebound requirement comes with a significant limitation: if you are seen leaving your home regularly — attending church, going to a family dinner, participating in community activities — you may be considered no longer homebound and may lose your eligibility for services.

For individuals recovering from stroke, Parkinson's, TBI, or other neurological conditions, that restriction can create a quiet, discouraging choice: stay home and keep your therapy, or participate in your life and risk losing it.

What is mobile outpatient therapy?

Mobile outpatient therapy is outpatient therapy — the same skilled, certified, goal-driven neurological rehabilitation we provide in our clinic — delivered in your home.

There is no homebound requirement. You are free to go to church, attend your grandchild's birthday party, run errands, and participate fully in your community. Your therapy eligibility is not affected by how often you leave the house.

At Salience, we bring our therapists — and when possible, our equipment — directly to you. You receive the same one-on-one, individualized, neuroplasticity-based treatment in your home that our clinic patients receive on site.

Who is mobile outpatient therapy for?

Mobile outpatient therapy is a strong option for individuals who:

Have transportation barriers that make regular clinic visits difficult or impossible.

Live with a caregiver who is not able to transport them but who wants them to receive skilled outpatient-level care.

Are in a stage of recovery where leaving home is possible but tiring — and where the energy spent traveling would be better used in the session itself.

Prefer the comfort and familiarity of their own environment, which research supports as a meaningful factor in neuroplastic learning.

Want their therapy environment to match their actual daily living environment — because the goal of recovery is ultimately to function better at home, in real life.
